Sudden cardiac death in sport: what cardiology says

Every time a headline reports a cardiac arrest at a local race, we all think the same thing. Here's what cardiology actually knows, without alarmism or embellishment.

The news appears, always in the same words: a 43-year-old runner, kilometre 18, some local road race. And for the rest of the day, as you lace up your trainers, there's a small voice at the back of your head asking the question nobody says out loud. Let's say it, and answer it with what cardiology knows, which is rather more than fits in a headline.

Age changes the whole conversation

The Spanish Society of Cardiology has reviewed on its blog a review published in the New England Journal of Medicine on cardiac arrest in athletes, and the first thing that brings order to the picture is age. Below 25, congenital, structural and electrical conditions dominate: cardiomyopathies and channelopathies, almost always things you're born with. Above that age, the lead role goes to something else, and something far more familiar: coronary artery disease.

Which means the headline you read and the fear you feel often aren't about the same problem at all. If you're 45, what concerns you isn't some vanishingly rare inherited arrhythmia: it's the ordinary cardiovascular risk factors your GP mentions every time they look at your cholesterol and blood pressure.

One detail stands out in contemporary studies: the most frequent post-mortem finding is the so-called "negative autopsy", a structurally normal heart. In other words, a good share of these cases points to a primary arrhythmic origin, something invisible because it leaves no mark on the tissue. That's uncomfortable to accept, but it's the truth available, and it beats a reassuring explanation someone made up.

The device that saves most lives isn't worn on your wrist

Here's the practical message, and it has nothing to do with heart-rate monitors or rings that measure your variability. What changes the outcome is the automated external defibrillator, and someone knowing how to use it fast.

Having defibrillators and structured response plans in place saves lives and remains the measure with the greatest real impact.

"Structured response plans" sounds like paperwork, but it means something very concrete: that your club, your sports centre or your race has a device, that it's where it should be, and that somebody knows what to do in the first three minutes. That saves more lives than any gadget you buy for yourself.

What about a medical screening?

The review is clear about the tools. Clinical history and physical examination, on their own, have low sensitivity: asking how you feel and listening to your chest doesn't get you far. An ECG does clearly increase diagnostic performance, and its findings then direct any further tests where needed: echocardiogram, MRI or CT. Genetic testing has also moved towards targeted use, particularly when the autopsy is negative.

What you won't find here is medical advice on whether you personally should get screened. That depends on your age, your family history, your symptoms and your story, and it's a conversation for a consulting room, not a blog. What you can take away is that the conversation with your doctor is worth having, and that turning up with your family history at hand makes it far more useful.

From banning to accompanying

The underlying shift of recent years is almost philosophical. For a long time, a cardiac diagnosis meant hanging up your trainers, full stop. Today the Spanish Society of Cardiology sums up the turn in a single sentence: we have moved from systematic prohibition to a return to sport in selected patients. It's decided case by case, shared between doctor and patient, and many go back to sport without a significant increase in events.

What is in your hands

  • If something feels off when you push hard — and you know perfectly well what's odd for you — don't tough it out or leave it until Monday. Get it checked.
  • Your family history is part of the clinical history, which is the first rung of any screening. Know it before you walk into the appointment.
  • Find out where the defibrillator is at your gym, your club or your sports centre. If you don't know, you've already learned something today.
  • Take a basic CPR course. It lasts a morning, and it's one of the few things you learn in the hope of never needing it.
  • Keep training. The balance of exercise for your heart isn't in dispute; what's debated is how to spot the exceptions in time.

What to take home

Sudden cardiac death in sport is infrequent, it isn't homogeneous, and it doesn't fall evenly across ages, sexes or disciplines. And above all: it isn't an argument against sport. Quite the opposite — training is one of the best decisions you can make for your health, and along the way it changes you on the inside, well beyond your heart.

So go for that run on Sunday. But first, check where your club keeps its defibrillator. It takes two minutes and, with a bit of luck, it'll be no use to you for the rest of your life.
